Subject access request


Personal information request

Please note that we may use approved and trusted third-party technology to process subject access requests. You may receive your completed request from Smartbox.ai, however these are being sent on behalf of London Borough of Hackney.


Please attach copies of identification and proof of address to your request. Processing of your request will begin once suitable ID has been received. We delete identity documents immediately after we have verified your identity.

Examples of acceptable name identification:
- driver's license
- passport
- birth certificate
Examples of acceptable proof of address:
- utility bill
- bank statement
- council tax bill
